Hi Doc Medical Center provides medical evaluations and documentation assistance for eligible FMLA-related medical leave requests. Appointments include medical review, documentation review, and provider completion of qualifying forms when medically appropriate.

Completion of paperwork does not guarantee employer approval. Forms are completed based on clinical evaluation and medical necessity.

Disabled Parking Placard Evaluation

$150

Hi Doc Medical Center provides medical evaluations for patients seeking a Florida disabled parking placard. During the appointment, a provider will review medical history, mobility limitations, and qualifying conditions to determine whether documentation is medically appropriate.

Pregnancy Parking Placard Evaluation

For qualifying pregnancy-related mobility limitations, Hi Doc Medical Center offers evaluations for temporary parking accommodation documentation. During the visit, a provider will review pregnancy-related symptoms, mobility concerns, and medical necessity.

Doctor's Note & Return-to-Work Evaluation

Hi Doc Medical Center provides brief medical evaluations for patients requesting doctor’s notes, work excuse documentation, school excuse notes, or return-to-work documentation.

A provider evaluation is required. Documentation is only issued when supported by clinical findings. Notes cannot be improperly backdated.

Please bring a valid photo ID along with any required forms — including employer FMLA paperwork, Florida DMV placard forms (HSMV 83039), or return-to-work documents — and any relevant medical records that support your request.
